What Is Intermediate Care?
Also known as reablement, intermediate care is a free, time-limited support service (usually up to six weeks) for individuals recovering after a hospital stay or sudden illness. It focuses on helping people regain daily living skills, avoid unnecessary readmissions, and ease back into home life with the right support.
The Four Types of Intermediate Care
🔄 Reablement – Support with everyday tasks like getting dressed, staying clean, preparing meals, and managing routines, all aimed at restoring confidence and independence.
🏡 Home-Based Care – Delivered at home by a multidisciplinary team, this type includes personal care, therapy, and health monitoring, ideal for more complex recovery needs, such as post-operative care.
🛏️ Bed-Based Care – Provided in a care home, hospital, or step-down facility for those who are temporarily unable to return home. It offers focused rehabilitation in a safe environment.
🚨 Crisis Response – Rapid-response care provided within hours of an emergency to stabilise a patient and potentially prevent hospital admission if safe alternatives exist.
